Job Description

 Work Schedule: Monday - Friday 8:00am - 5:00pm OR PRN Monday - Friday 8:00am - 8:00pm (your choice of days you would like to work at least two days per week)

 

Responsibilities:

  • Perform initial assessments to prioritize patient care based on the severity of their condition.
  • Administer vaccines, medications, or other injectable treatments as required.
  • Provide immediate care in emergency situations, following established protocols.
  • Develop a strong, supportive relationship with patients to foster trust and cooperation.
  • Evaluate conditions like cardiovascular disease, diabetes, and asthma.
  • Identify socio-economic and ecological factors affecting patient wellness and treatment.
  • Create and implement effective treatment plans tailored to individual patient needs.
  • Link patients to appropriate resources such as family support, treatment programs, and emergency assistance.
  • Recognize and address obstacles to following treatment plans.
  • Assist patients in overcoming challenges to adhere to their treatment regimen.
  • Monitor and follow up on patient care to improve outcomes.
  • Work with providers, medical assistants, social services, and registration staff to ensure comprehensive care.
  • Coordinate with team members regarding patients’ social and mental health needs.
  • Utilize multi-modal educational methods to manage chronic conditions, such as blood pressure follow-up, family planning, and immunization.
  • Oversee quality control checks for glucometers and other waived testing.
  • Ensure compliance with infection control procedures.
  • Provide clear and comprehensive discharge instructions to patients.
  • Monitor and address patient use of health center services and barriers to accessing adequate care.
  • Execute and monitor data collection for institutional and federal regulations, using electronic or manual systems.
  • Participate in continuing education and attend relevant meetings.
  • Maintain accurate, up-to-date medical records as per policy and standards.
  • Assist with the supervision and delegation of tasks to LPNs and MAs as per the nursing scope of practice.
  • Identify areas for clinical improvement and develop action plans in consultation with appropriate personnel.
  • Conduct home visits for patients unable to attend appointments, if appropriate.
  • Perform other duties as assigned to support overall patient care and team functions.

Job Requirements

Qualifications:

  • Graduation from an accredited school of nursing with a minimum ADN.  Must obtain BSN within five (5) years of hire into the position.
  • Current active DC License as a Registered Nurse.
  • Current CPR Certification.
  • If ADN, two (2) years of clinical experience.
  • If BSN, one (1) year of clinical experience.
  • Clinical expertise may include any of the following: emergency room, mental and behavioral health, women’s health, pediatrics, infectious disease, community or public health, polypharmacy teaching, care coordination, discharge planning, triage, and chronic disease care management.
  • Additional work experience may include any of the following: teaching, health promotion, social and ecological determinants of health, monitoring and evaluation of health outcomes, working with diverse populations, family studies, and outreach. 
  • Professional skillsets including but not limited to communication, conflict resolution and management, and inter-professional teamwork.
  • Ability to work independently, to supervise and to work in a team-oriented environment.
  • Excellent interpersonal and written communication skills.
  • Must be well organized, flexible and action-oriented.
  • Demonstrated ability to effectively work and communicate with diverse populations.
  • Demonstrated proficiency with business software (e.g. Microsoft Office Suite).
