Job Summary:
We are seeking a dedicated Registered Nurse to join our clinical health care team.
The ideal candidate will be responsible for hands-on patient care and providing support to patients and their families throughout their healthcare journey.
Days/Hours:
Monday to Friday - 8a to 5p - no weekends or three 12 hour shifts, 8a to 8p - Monday to Friday - no weekends.
Duties:
- Conduct diagnostic evaluations and assessments to develop individualized care plans
- Collaborate with healthcare providers to ensure comprehensive patient care
- Manage patient cases in a clinic setting.
- Monitor patient progress and outcomes, adjusting care plans as needed
- Educate patients on treatment plans, medications, and self-care techniques
- Maintain accurate medical records and documentation
Skills:
The ideal candidate should possess the following skills:
- Proficiency in clinic, internal and physiology knowledge
- Experience in outpatient settings and primary care
- Ability to work with diverse patient populations including geriatrics
- Familiarity with medical records management
- Knowledge of case management principles
