Job Description

Responsibilities:

  • Routine tasks such as intake, triage, injections, and emergency care.
  • Build therapeutic relationships, assess chronic and acute health conditions (e.g., cardiovascular disease, diabetes, asthma), and address socio-ecological health barriers.
  • Develop treatment plans and link patients to appropriate support systems.
  • Work with the healthcare team (providers, medical assistants, social services) to ensure comprehensive patient care.
  • Help manage chronic illnesses and provide education on topics such as blood pressure, family planning, and immunizations.
  • Coordinate testing (e.g., glucometers) and ensure compliance with infection control protocols.
  • Ensure accurate and timely documentation of patient records, and adhere to regulatory requirements.
  • May oversee medical assistant staff, if required.
  • Facilitate home visits for patients unable to attend in-person appointments.

 

Job Requirements

Qualifications:

  • ADN (Associate Degree in Nursing) with 2 years of clinical experience, or BSN (Bachelor of Science in Nursing) with 1 year of clinical experience.
  • Active DC Registered Nurse license.
  • CPR certification.
  • Relevant clinical expertise in areas like emergency care, mental health, pediatric, community health, chronic disease management, and care coordination.
  • Strong interpersonal and written communication skills.
  • Ability to work independently and as part of a team.
  • Ability to handle multiple tasks efficiently in a fast-paced environment.
  • Experience working with diverse populations.
  • Ability to sit, stand, walk, and carry light items (books, files).
  • Visual acuity for detailed tasks and prolonged computer use.
  • Some stressful situations due to time-sensitive tasks.
  • Must be capable of working independently and managing stressful situations.
  • Must maintain health assessments (e.g., PPD or chest X-ray results).
  • Adhere to all safety protocols and community healthcare standards.
  • Flexibility to cover absences or peak work periods.
