Job Description

The role will be responsible for ensuring compliance with regulations and industry standards related to intellectual and developmental disabilities (IDD) services. Must be a DC resident!

 

Responsibilities:

  • Conduct in-person home visits to assess individuals with IDD, including interviewing clients, caregivers, and family members to gather relevant information regarding the individual's physical, emotional, social, and educational needs.
  • Observe and assess the living environment to determine accessibility, safety, and suitability for the individual’s needs.
  • Identify any gaps in care, safety concerns, or areas that may require intervention or improvement.
  • Use assessment tools and standardized questionnaires to gather data on the client’s health, living conditions, and social needs.
  • Accurately document the findings from home visits, including detailed reports on the individual's condition, environment, family support, and any immediate needs or concerns.
  • Complete assessments and create care plans or service recommendations based on the information gathered.
  • Prepare and submit assessment reports and recommendations to relevant service providers, healthcare professionals, or case managers.
  • Maintain up-to-date records in electronic systems and ensure compliance with legal and ethical standards, including confidentiality and privacy requirements (e.g., HIPAA).
  • Work collaboratively with healthcare providers, social workers, caregivers, and other service providers to ensure comprehensive care planning for individuals with IDD.
  • Participate in case conferences or team meetings to discuss assessment findings and develop coordinated plans for care and services.
  • Provide ongoing support and follow-up services to individuals and their families to ensure that care plans are implemented effectively.
  • Build rapport with individuals with IDD and their families, providing compassionate and empathetic support throughout the assessment process.
  • Offer guidance to families and caregivers about available resources, including community services, financial support, or healthcare services.
  • Address questions or concerns raised by families or caregivers during the home visit and provide appropriate referrals when necessary.
  • Act as an advocate for individuals with IDD by ensuring they receive the services and support they need, and guiding families in navigating the complex systems of healthcare and social services.
  • Assist clients in accessing resources such as special education services, Medicaid, home health services, or other public assistance programs.
  • Provide referrals to specialized services as necessary (e.g., therapy, counseling, vocational training).
  • Ensure that all assessments and documentation are completed in compliance with state and federal regulations, including ensuring confidentiality, informed consent, and privacy laws.
  • Adhere to the agency’s ethical guidelines and policies when interacting with clients and families.
  • Track assessment progress and follow up with clients and caregivers as needed to monitor any changes in the individual’s condition or living environment.
  • Provide feedback to supervisors or case managers about any urgent concerns or changes in the client’s needs.

Job Requirements

Qualifications:

  • Bachelor’s degree in Social Work, Nursing, Psychology, Special Education, or a related field. A Master’s degree is a plus.
  • At least 1-2 years of experience in social work, healthcare, or direct care for individuals with intellectual and developmental disabilities (IDD).
  • Experience conducting home assessments or working in a similar role with individuals who have disabilities or complex care needs.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and families.
  • Ability to assess and document care needs in a detailed and accurate manner.
  • Familiarity with assessment tools for individuals with IDD, and knowledge of relevant care planning strategies.
  • Problem-solving skills to identify areas of need and suggest solutions or appropriate services.
  • Experience with case management and knowledge of resources available to individuals with IDD.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
  • Experience with electronic health records (EHR) or case management systems.
  • Ability to use assessment software or documentation tools.
  • Experience with individuals with complex needs, including autism, cognitive impairments, or physical disabilities.
  • Knowledge of local resources in Washington, D.C., including services through Medicaid, social services, and local advocacy organizations for people with disabilities.
  • Experience in family counseling or caregiver support.
  • This position requires frequent travel to clients' homes within the Washington, D.C. area, so reliable transportation is necessary.
  • Strong ability to connect with clients and families in sensitive and sometimes difficult situations.
  • Ability to accurately assess, document, and report findings from home visits.
  • Ability to manage home visit schedules and documentation efficiently.
  • Strict adherence to privacy standards and ethical conduct in all interactions with clients and their families.
  • Willingness to adapt to different client needs and changes in schedules.
