We are seeking a skilled and compassionate IDD Case Manager to join our team and support individuals with intellectual and developmental disabilities in residential settings. This role is ideal for a dedicated professional with hands-on experience who is committed to advocating for individuals’ well-being, independence, and quality of life.
Location: Washington, DC (Must be a DC resident)
Schedule: Monday–Friday, 8:30 AM – 5:00 PM
Work Model: Remote with required weekly site visits throughout Washington, DC
Conduct comprehensive assessments to evaluate residents’ needs, strengths, and areas for support
Develop, implement, and monitor individualized service plans in collaboration with interdisciplinary teams
Interview residents and family members to ensure person-centered care and inclusion of their perspectives
Maintain accurate and timely documentation of resident progress and changes in needs
Coordinate with external agencies and funding sources, including Medicaid and Medicare, to ensure proper documentation and compliance
Advocate for residents’ rights, community inclusion, and access to appropriate services
Provide guidance and training to staff on best practices for supporting individuals with developmental disabilities
Stay current on industry standards, best practices, and advancements in developmental disability services

Bachelor’s degree in psychology, Social Work, Special Education, or a related field
3–5 years of direct experience working with individuals with developmental disabilities in a residential setting
Knowledge of Medicaid and Medicare regulations related to service delivery
Strong communication and interpersonal skills for collaboration with residents, families, and care teams
Excellent organizational skills and attention to detail
Proficiency with electronic documentation and record-keeping systems
Ability to adapt in a dynamic environment and manage multiple priorities
Must pass a drug screening and background check
Must be a current resident of Washington, DC
