Job Description

We are seeking a skilled and compassionate IDD Case Manager to join our team and support individuals with intellectual and developmental disabilities in residential settings. This role is ideal for a dedicated professional with hands-on experience who is committed to advocating for individuals’ well-being, independence, and quality of life.

Location: Washington, DC (Must be a DC resident)
Schedule: Monday–Friday, 8:30 AM – 5:00 PM
Work Model: Remote with required weekly site visits throughout Washington, DC

 

Key Responsibilities

  • Conduct comprehensive assessments to evaluate residents’ needs, strengths, and areas for support

  • Develop, implement, and monitor individualized service plans in collaboration with interdisciplinary teams

  • Interview residents and family members to ensure person-centered care and inclusion of their perspectives

  • Maintain accurate and timely documentation of resident progress and changes in needs

  • Coordinate with external agencies and funding sources, including Medicaid and Medicare, to ensure proper documentation and compliance

  • Advocate for residents’ rights, community inclusion, and access to appropriate services

  • Provide guidance and training to staff on best practices for supporting individuals with developmental disabilities

  • Stay current on industry standards, best practices, and advancements in developmental disability services

Job Requirements

Qualifications

  • Bachelor’s degree in psychology, Social Work, Special Education, or a related field

  • 3–5 years of direct experience working with individuals with developmental disabilities in a residential setting

  • Knowledge of Medicaid and Medicare regulations related to service delivery

  • Strong communication and interpersonal skills for collaboration with residents, families, and care teams

  • Excellent organizational skills and attention to detail

  • Proficiency with electronic documentation and record-keeping systems

  • Ability to adapt in a dynamic environment and manage multiple priorities

  • Must pass a drug screening and background check

  • Must be a current resident of Washington, DC
