This is a long term contract nursing position.   

Location: Homeless Shelter, Washington, DC

Schedule: Monday to Friday, 8:30 AM to 4:00 PM

Job Summary:

We are seeking a dedicated and detail-oriented Case Manager, Registered Nurse to join our team at a homeless shelter in Washington, DC. This role primarily focuses on administrative duties with an emphasis on patient advocacy and care coordination. The ideal candidate will possess strong organizational skills, proficiency with computer systems, and prior supervisory experience. This position involves monitoring patient care, scheduling and following through with doctor visits, and leading other staff members.

Key Responsibilities:

• Patient Care Monitoring:
  • Oversee and ensure the quality of patient care.
  • Maintain detailed records of patient health and treatment plans.
• Scheduling and Coordination:
  • Schedule and monitor doctor visits for patients.
  • Ensure follow-through on medical appointments and treatments.
• Patient Advocacy:
  • Serve as a patient advocate, ensuring patients receive the necessary care and support.
  • Communicate effectively with healthcare providers and patients.
• Staff Leadership:
  • Lead and supervise other staff members, providing guidance and support.
  • Coordinate team efforts to deliver comprehensive care to patients.
• Administrative Duties:
  • Utilize computer systems for documentation and scheduling.
  • Maintain organized and accurate records.

Qualifications:

• Licensure:
  • Must hold a current DC nursing license.
• Experience:
  • Previous supervisory experience required.
  • Experience working with vulnerable populations, particularly in a homeless shelter or similar environment.
• Skills:
  • Exceptional attention to detail.
  • Strong organizational and time management skills.
  • Proficiency with computer systems and software applications.
  • Excellent communication and interpersonal skills.
