Our firm is seeking a compassionate and organized Admissions Coordinator to join our Health Care Center team. This vital role is responsible for managing all aspects of resident admissions, maintaining census targets, and building strong relationships with referral sources, including hospital discharge planners and insurance networks.

Key Responsibilities
Admissions Coordination
Manage all admissions to the health care facility, including reviewing referrals, collecting medical documentation, verifying insurance, and obtaining necessary authorizations.
Serve as a primary contact for prospective residents and their families—providing information, explaining services and requirements, and ensuring a smooth and supportive admission experience.
Build and maintain strong relationships with hospital discharge planners, physician practices, insurance networks, assisted living communities, and other referral sources.
Confirm Medicare and insurance eligibility; coordinate with case managers and financial departments as needed.
Track hospitalized residents and coordinate transitions back to the facility when appropriate.
Ensure resident rooms are properly prepared in collaboration with facilities and environmental services teams prior to admission.
Act as the point of contact for Medicare Advantage and HMO/PPO plans—handling benefit verification, pre-authorizations, and ongoing communication with case managers and families.
Administrative Support
Maintain and update the Plan-of-Care schedule to ensure timely inclusion of new admissions.
Enter and manage resident census data in the register, updating in real-time and distributing reports weekly.
Manage inventory and ordering of office supplies for both the health care center and nursing administration.
Sort and distribute incoming mail to the appropriate departments, staff, or residents.
Qualifications
Bachelor’s degree in Healthcare, Social Work, or a related field preferred.
Working knowledge of Medicare and commercial insurance plans, with the ability to clearly communicate coverage details and out-of-pocket responsibilities.
Basic understanding of medical diagnoses and the scope of services provided in a skilled nursing or post-acute care setting.
Excellent verbal and written communication skills with a warm, professional, and compassionate approach.
Strong organizational skills with the ability to manage multiple tasks and priorities efficiently.
