Job Description

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team serving vulnerable populations. This position will play a critical role in supporting the health and well-being of our clients by providing basic medical care, monitoring chronic conditions, and collaborating with our multidisciplinary team.

 

Responsibilities:

  • Provide direct patient care including medication administration, wound care, and basic assessments
  • Monitor and document patient conditions, vital signs, and responses to care
  • Assist with chronic disease management and health education
  • Coordinate care and referrals with case managers, mental health professionals, and external medical providers
  • Maintain accurate and timely documentation in medical records
  • Respond to medical emergencies and provide first aid as needed
  • Uphold infection control standards and support a safe, respectful environment
  • Communicate compassionately with a diverse and sometimes high-needs population

 

Shift: Tuesdays and Thursdays 7a-7p

Job Requirements

Qualifications:

  • Active LPN license in the state of DC
  • At least 2 years of nursing experience (experience with underserved populations preferred)
  • Strong clinical assessment and communication skills
  • Ability to remain calm and effective in crisis or emergency situations
  • Comfortable working independently and as part of a multidisciplinary team
  • Passion for serving marginalized communities and addressing social determinants of health
  • Experience in community health, shelters, or similar environments
  • Familiarity with mental health and substance use issues
  • EMR or EHR experience
